I am the same. I only wear underwear to the dermatologist.
My other doctors don't have me take my pants off, except the urologist who wants everything off below the waist.
WHY, when they want to see every part of you?
My dermatologist's assistant always tells me to strip to my underwear. When the doctor arrives he checks the rest of my body then looks inside my underwear without removing it. I have told him I spend time nude in the sun so he is aware of it.

I believe a dermatologist does a better checkup when the patient is nude instead of looking inside their underwear. Ive had both kinds and one who didnt even check that area! Only time I went to that practice.

Dare I say that the dermatologist was not doing his job properly.
When I had a full body wax I stripped naked. Then the lady laid a towel across my intimate parts while she did the rest of me. I asked why. She knew I'm a nudist. And she'd be removing the towel later to do my intimate waxing. Then she certainly saw everything, and handled it all before examining it closely under a bright light and magnifying glass (maybe my bits need a magnifying glass lol)
