Why wear underwear?

I don't wear underwear at the doctor's office. In fact, if I'm there for a physical, then I strip after the nurse leaves the room and before the doctor enters. He's going to look at everything anyway. I wouldn't be the first time he's seen a naked man. When I had a female GP, I did the same thing.
Less than 5 years ago I worked in an office with several women on staff, I managed a department independently but would have occasional contact with the other staff.
The dress code was business casual which for me consisted of dress jeans or kakis, dress shirts or polos, (side note, I never wore underwear, I was always commando, thinking no would notice or care).
One day while I was in the staff filled main office, wearIng a polo shirt and kakis, I noticed several women in a group looking in my direction, finally one of the bolder women, representing the group, walked towards me and asked, Are you wearing underwear?... without missing a beat, feeling if she had the right to ask, so did I, I responded, Are you? she was so surprised with my quick response and got so embarrassed, the topic was never approached again... I guess going commando in the office was noticed!
Ive always been fascinated by guys that go commando at work. Its one thing to do it on ur personal time, running errands, gym, but doing it at work adds such an erotic new layer to the concept. When Im in the lockers, i notice guys dress and put on their dress slacks without underwear and I totally get aroused by this. Knowing they bare penis is covered by such thin fabric during that workday. Ive gotten so used to going commando that the visibility of it in public rarely bothers me anymore. I still remember wearing some nylon sweat pants, commando, in front of a buddy of mine, he asked if i was wearing underwear. I told him no. When i got home, i looked at myself in the mirror still wearing the sweats and noticed how visible my penis outline was, the head was clearly seen as well as the shaft pressed up against the thin fabric.
I havent worn underwear since I was old enough to dress myself. Except for 8 weeks in boot camp back in 81. Even when in uniform, I went commando. Which sometimes got interesting wearing whites in the rain.
The underwear that I do have are boxer shorts which I'll wear for yard work.
Had my semi-annual dermatologist appointment yesterday and wore underwear for the first time since I tossed my last brief swimsuit.
Felt more confining than ever.
Considering tossing my only pair of underwear and going without next time.
I must admit, however, that 3 times in the past I have done this and wound up buying a pair when my next appointment arrived.

I can relate. On the other hand, have been bare down there for 30 plus years and never had a negative reaction from a Dermatologist.

My dermatologist either peeks quickly inside my underwear or has me pull it down and takes a very quick look. He acts as if he would be uncomfortable if I were nude during the entire examination.
