The certification instructions said to download and print the certification document but I did not see
a link for this purpose. How do I find it? Thanks for the help.

On that page the words "Print out the certification document" is the link. Try clicking that line and a new tab opens with a printable page that is formated just like the one in the pictures, but with your username.
In a misguided attempt to make the list more pretty, TT1 hid the formating that made the link obvious.
