Dermatologist

I had an appointment this past week with a new dermatologist. I've had a couple of cases of skin cancer in the past so I need regular check ups and my previous doctor retired when covid hit.
So I get to my appointment and the nurse told me to strip to my briefs and put on a paper gown. I told her I don't wear underwear, and I'm a nudist so I don't need a gown. She says no problem.
The doctor comes in while I'm sitting on the table naked and comments on my overall tan. I tell her I'm a nudist and spend lots of time outside naked. She tells me that she has visited a couple of clothing optional beaches and she appreciates the fact that I'm willing to do the exam naked as it makes it easier for her. She said most of her patients are much too modest to strip completely.
During the exam she found a spot that needed to be removed on my back and she asked if I wanted to put my pants on for the procedure. I said I was OK not getting dressed if she was and she replied that was fine.
It was refreshing to find a doctor and nurse that were very comfortable with nudity in the exam room.

My dermatologist's assistant told me to strip to my underwear, so that is what I did.
After I got rid of all of my underwear, I wore a solid swim brief under my pants or shorts and left it on.
The doctor always did look inside when I said he should because I sun and swim nude.
After several years, one of these conversations lead me to ask how he handles patients that don't wear underwear.
He said he actually thought it was better to examine patients nude, but doesn't ask for it because he know it would make too many people uncomfortable.
Should have asked sooner.

As I was getting ready to leave:
Wife "Aren't those shorts too short to wear to the doctor's" (FYI, they had a 4 or 5'" inseam).
Me "It's the dermatologist. I'll be naked when he sees me".
No further comments from wife.
